Mein Vorschlag zur Therapie einer nCoV-Infektion/Coronavirusinfektion - endosomale pH-Wert Manipulation mittels Chloroquin...

liebe Freunde der Liebe und des Glücks,
liebe Mitleser,
im folgenden Beitrag möchte ich einen bislang bei Coronavirus infizierten nicht erprobten Therapievorschlag zur Behandlung der 2019-nCoV-Erkrankung machen und als erster auch in der Blockchain direkt publizieren und verankern. Mir scheint es wichtig auf logisch rationalem Wege alle denkbaren Therapieoptionen bei der Coronavirus-Infektion durch zu spielen - angefangen im Vorfeld der Interaction zwischen Virus und Wirtszelle bis hin zur postranslationalen Prozessierung der Virusbestandteile in der Zelle im Rahmen der Virusmehrung innerhalb der Zelle und deren späterer Ausschleusung aus der Zelle - sprich dem budding der neu zusammengesetzten Viruspartikel.
Im Rahmen meiner stundenlangen Recherchen und dem Studium der Virusliteratur in den vergangenen Tagen habe ich mir auf Grund der strukturellen Ähnlichkeit zwischen dem Lassavirus und dem Coronavirus bei der Ausgestaltung der Virushülle im Hinblick auf die Fusionseigenschaften unter anderem eine Promotion zu Gemüte geführt.
Dabei fiel mir auf, dass es zwischen dem Lassavirus und dem nCoV-Virus im Hinblick auf die übergeordneten Mechanismen der Virusreplikation (Fusion mit der Wirtszelle, Endozytose, Translation der Virus RNA in Virusproteine, posttranslationales Prozessing der neugebildeten Viruseiweiße und deren Exozytose via "Budding" - sprich Knospung aus der Zelle - kaum nennenswerte Unterschiede gibt.
Bemerkenswert und weiter verfolgenswert fand ich dabei die Erkenntnis aus der Lassaforschung, dass die Vermehrung des Lassavirus in der Wirtszelle durch Veränderung des ph-Wertes in den Endosomen der Wirtszelle blockiert bzw. gehemmt werden kann.

Was sind Endosomen - und wofür braucht das Coronavirus diese?

Endosomen sind - um es kurz zu machen - Zellbestandteile die normalerweise wie ein Verdauungsapparat funktionieren, weshalb sie auch als Lysosomen bezeichnet werden. Der ph-Wert von Endosomen liegt normalerweise im sauren Bereich zwischen 5.8 und 6.4. Nähere Information findet ihr unter Punkt 13 der Quellennachweise. Im Rahmen der Virusreplikation dienen die Endosomen den viralen Infektionserregern, wie dem tödlichen Lassa-Virus, dessen Virushülle ebenfalls wie das Coronavirus nCoV virale Spikes aufweist, als "Tor zum Zellinneren" einer Wirtszelle, welches die Virus RNA wie in einem Fahrstuhl in die zellulären Produktionhallen eines Zellbetriebes transportiert, wo dann später die neuen Viruspartikel synthetisiert werden.
Dieser als Endocytose bezeichnet Prozess, der nach der Fusion des Virus mit der Zellmembran von statten geht ist offenbar zumindest beim Lassavirus abhängig vom ph-Wert in den Endosomen der Wirtszelle. Ist dieser ph-Wert niedrig - sprich befindet sich dieser im sauren Bereich, so begünstigt diese Konstellation eine Fusion der Viruspartikel mit der Wirtszelle und damit die Endozytose der todbringenden Erreger - sprich den Eintritt des Virus in die Zelle.
Hebt man den ph-Wert in den Endosomen der Zelle jedoch an, so wird die Fusion des Lassavirus mit der Wirtszelle offenbar empfindlich gestört und gehemmt. Die Anhebung des ph-Wertes in den Endosomen der Zelle kann unter andrem mit dem in der Humanmedizin weit verbreiteten Medikament Chloroquin hergestellt werden. Dieses Medikament wird dabei heutzutage gerne in der Rheumatherapie, wie auch in der Malariaprophylaxe eingesetzt.
Chloroquin und Ammoniumchlorid sowie die carboxylischen Ionophoren Monensin und Nigericin stellen lysosomotrope schwache Basen dar, die folglich den endozytotisch vermittelten Viruseintritt in eine Zelle hemmen können. [12. S.18]
Sucht man in den medizinischen Datenbanken nach vergleichbaren Prozessen auf endosomaler Ebene bei den Coronaviren, so findet man eine Dissertation aus der Charité in Berlin, welche experimentell am MHV-Coronavirus den S-Protein vermittelten und wie beim Lassa-Virus auch Clathrin abhängigen Endozystose Prozess in vitro bei Experimenten mit Mauszellen mit lysomotropen Substanzen - ganz in Analogie zum Lassa-Virus - hemmen konnten. [11]
Ursächlich für die Hemmung des Eintritts des Coronavirus in die Wirtszelle ist dabei offensichtlich die Erhöhung des ph-Wertes in den Endosomen, welche offenbar zu einer unumkehrbaren - sprich irreversiblen Strukturveränderung in dem so wichtigen Protein S des viralen Fusionseiweiß führt und somit offenbar den Verlust der Infektivität des Coronavirus zur Folge zu haben scheint.
Diese Erkenntnis ist bemerkenswert - und das zugleich in zweierlei Hinsicht:
  1. Es zeigt sich das offenbar - zumindest experimentell bei in vitro Versuchen - der ph-Wert der Endosomen der Wirtszelle am Zielorgan der Viren nicht nur beim Lassavirus, sondern auch bei Coronaviren - eine entscheidende Rolle beim Eintritt des Virus in die Wirtszelle zu spielen scheint. Demnach kann durch die Gabe von lysomotropen Substanzen wie beispielsweise Chloroquin modellhaft an Mauszellkulturen einer Coronavirusinfektion die Infektiosität eines Coronavirus blockiert oder zumindest gehemmt werden und damit die direkte Schädigung weiterer Wirtszellen offenbar vermieden werden kann. Und dies unabhängig davon ob es sich dabei um ein Arenavirus wie dem Lassavirus handelt oder um ein Coronavirus. Demnach scheinen - zumindest einige Viren - den gleichen Entrymechanismus speziesübergreifend beim Eintritt in eine Wirtszelle auf endosomaler Ebene zubenutzen, was die Möglichkeit zur Entwicklung von lysosomotropen Breitbandvirustatika ermöglichen sollte.
  2. Durch Erhöhung des pH-Wertes in den Lysosomen/Endosomen der Wirtszelle mittels lysomotroper Substanzen lässt sich scheinbar eine forcierte direkte Organschädigung durch Endozystose von Viruspartikeln auf der Eintrittsebene in die Wirtszelle im Falle eines SARS-Modell-Coronavirus (MHV-59-Coronavirus) irreveribel unterbinden.
Fazit: Auf Grundlage dieser gewiss noch dünnen Datenlage einer Blockade des Viruseintritts von Coronaviren durch lysosomotrope Substanzen in präklinischen in vitro Modellen, ist eine virustatische Therapie mittels Chloroquin (Handelsname: Resochin) zur Therapie der nCoV-Infektion bei Schwererkrankten im Rahmen klinischer Studien zu erwägen um weitere Ogranschäden durch unlimitierte Replikation der Viren zu unterbinden und damit den Tod von Infizierten zu verhindern. Darüber hinaus gilt es zu erforschen, ob lysomotrope Therapeutika, die der ph-Wert Anhebung in den Lysosomen (Endosomen) dienen auch zur Therapie anderer viraler Erkrankungen und Infektionen verwendet werden können, da die Hemmung der viralen Replikation durch Manipulation des ph-Wertes in den Endosomen offenbar nicht nur bei Lassaviren zu funktionieren scheint, sondern auch bei Coronaviren - zumindest in den in vitro Versuchskonstellationen.

Dear readers,
In the following article I would like to make a previously untested therapy proposal for coronavirus for the treatment of the 2019 nCoV disease and be the first to publish and anchor it directly in the blockchain. It seems important to me to play through all conceivable therapy options for coronavirus infection in a logically rational way - starting with the interaction between virus and host cell up to post-translational processing of the virus components in the cell as part of the virus multiplication within the cell and its subsequent removal out of the cell - that is, the budding of the newly assembled virus particles.
In the course of my hours of research and studying the virus literature in the past few days, due to the structural similarity between the Lassavirus and the Coronavirus, I got a doctorate regarding the fusion properties with regard to the fusion properties.
It struck me that there is a difference between the Lassavirus and the nCoV virus with regard to the higher-level mechanisms of virus replication (fusion with the host cell, endocytosis, translation of the virus RNA into virus proteins, post-translational processing of the newly formed virus proteins and their exocytosis via "budding" - ie budding from the cell - there are hardly any noteworthy differences.
What I found remarkable and worth pursuing was the knowledge from Lassa research that the proliferation of Lassavirus in the host cell can be blocked or inhibited by changing the pH in the endosomes of the host cell.

What are endosomes - and what does the coronavirus need them for?

In short, endosomes are cell components that normally function like a digestive system, which is why they are also called lysosomes. The pH of endosomes is usually in the acidic range between 5.8 and 6.4. You can find more information under point 13 of the references. In the context of virus replication, the endosomes serve the viral infectious agents, such as the deadly Lassa virus, whose viral envelope, like the coronavirus nCoV, has viral spikes, as a "gateway to the cell interior" of a host cell, which carries the virus RNA like an elevator into the cellular production halls a cell plant, where the new virus particles are later synthesized.
This process, known as endocytosis, which takes place after the virus merges with the cell membrane is apparently dependent, at least in the Lassavirus, on the pH value in the endosomes of the host cell. If this pH value is low - in other words, it is in the acidic range, this constellation favors a fusion of the virus particles with the host cell and thus the endocytosis of the deadly pathogen - that is, the entry of the virus into the cell.
However, if the pH in the endosomes of the cell is raised, the fusion of the Lassavirus with the host cell is apparently sensitively disturbed and inhibited. The increase in the pH value in the endosomes of the cell can be achieved using the drug chloroquine, which is widely used in human medicine. This drug is often used in rheumatism therapy as well as in malaria prophylaxis.
Chloroquine and ammonium chloride, as well as the carboxylic ionophores monensin and nigericin, are lysosomotropic weak bases, which can consequently inhibit the endocytotic virus entry into a cell. [12th p.18]
If you search in the medical databases for comparable processes at the endosomal level with the coronaviruses, you will find a dissertation from the Charité in Berlin, which experimentally mediated the S protein on the MHV coronavirus and, as with the Lassa virus, also dependent on the clathrin-dependent endocystosis process in in vitro in experiments with mouse cells with lysomotropic substances - in analogy to the Lassa virus. [11]
The reason for the inhibition of the entry of the coronavirus into the host cell is obviously the raise of the pH in the endosomes, which apparently leads to an irreversible - i.e. irreversible - structural change in the so important protein S of the viral fusion protein and thus apparently the loss of infectivity of the corona virus appears to result.
This finding is remarkable - and at the same time in two ways:
  1. It appears that - at least experimentally in in vitro experiments - the ph value of the endosomes of the host cell at the target organ of the viruses not only in Lassavirus but also in coronaviruses - seems to play a decisive role in the entry of the virus into the host cell , Accordingly, the infectivity of a coronavirus can be blocked or at least inhibited by the administration of lysomotropic substances such as chloroquine, for example, to mouse cell cultures of a coronavirus infection, and the direct damage to further host cells can thus obviously be avoided. And regardless of whether it is an arena virus like the Lassavirus or a corona virus. According to this, at least some viruses seem to use the same entry mechanism across species when entering a host cell at the endosomal level, which should make it possible to develop lysosomotropic broadband virus drugs.
  2. By raysing of the pH in the lysosomes / endosomes of the host cell by means of lysomotropic substances, an apparently forced direct organ damage by endocystosis of virus particles at the entry level into the host cell in the case of a SARS model coronavirus (MHV-59 coronavirus) prevent irreversible.
Conclusion: On the basis of this still very thin data base of a blockage of the entry of coronaviruses by lysosomtopic substances in preclinical in vitro models, a virustatic therapy with chloroquine (Resochin) for the therapy of nCoV infection in seriously ill patients should be considered within the scope of clinical studies to prevent further ogran damage and death through unlimited replication of the viruses. In addition, it is important to investigate whether lysomotropic therapeutics that serve to raise the pH in the lysosomes (endosomes) can also be used to treat other viral diseases and infections, since the inhibition of viral replication by manipulating the pH in the endosomes apparently not only seems to work with lassaviruses, but also with coronaviruses - at least in the in vitro experimental constellations.
